Dr. Wallace's Quick Screening Rules:

• Patient cannot have multiple missing teeth.
• Patient must have good oral hygiene and no active cavities.
• Patient cannot have active gum disease.
• Patient cannot have extreme jaw misalignments or severe over/under bites.
• Patient must be mature enough to consistently wear their aligners for 22hr/day and not lose them.​

1. First we will need a signed informed consent form and you should read the instructions for use. 
2. We must address costs. Total Cost is $5,575.
   1. Setup appointment $250 (Non-refundable) - impressions, xrays, photographs, measurements.
   2. $1,750 before aligners can be made.
   3. $3,575 before you receive the 3rd set of trays.
